The transition from hospital to home is a high-risk period for patients. Providing clear information about “what to watch for” reduces post-discharge anxiety and prevents unnecessary readmissions. Notably, this measure asks specifically whether the patient’s family or caregiver received enough information, recognizing that the people supporting recovery at home are often the ones who first notice a warning sign. When families and caregivers are equipped with specific symptom knowledge, they become active partners in the patient’s recovery and safety. Because the standard is “enough” information rather than simply information provided, success depends on confirmed understanding, not just a discharge packet handed over at the door.
