This measure captures something that every patient instinctively notices: whether the people caring for them actually know their story. When a patient watches a nurse enter the room and immediately ask questions that were already answered two shifts ago, or sees a physical therapist glance at the whiteboard with confusion, trust erodes. The HCAHPS question ‘How often were doctors, nurses, and other hospital staff informed and up to date about your care?’ is a direct measure of care coordination perceived by the patient, and it is one of the most operationally complex dimensions to improve because it requires every member of the care team to communicate consistently, not just some of the time.
